Barb

Barb was residing in a short-term rehabilitation facility outside of the Lakeland service area when she enrolled with Lakeland Care, Inc. (LCI).

She wanted to be able to return to her home within the community. However, progress in her rehabilitation stopped, and the facility was unsure of a long-term plan for Barb. She required assistance with Activities of Daily Living including using a lift for transfers and a wheelchair for mobility. With the help of LCI, Barb was transferred to a rehabilitation center closer to home. Barb worked hard in therapy, and she progressed tremendously at the new facility. Barb was able to discharge home with minimal supports, and she has been doing very well living in her own apartment ever since.

“I was able to move back to Fond du Lac and into my apartment again with the help of Lakeland,” said Barb. “(Lakeland) listened to what was important to me and worked with me to help meet my goals. I can’t say ‘Thank you!’ enough for everything they’ve done. They also financially helped by funding my rehab stay which enabled me to increase independence and return to my apartment.”
