Lakeland Care offers the Family Care Program in the following counties: Brown, Calumet, Door, Florence, Fond du Lac, Forest, Kewaunee, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Winnebago, and Wood.

Lakeland Care offers tribal care management for the Menominee Tribe and Oneida Nation. 

We have offices located throughout our service region in Appleton, Crivitz, Fond du Lac, Green Bay, Manitowoc, Marinette, Rhinelander, Shawano, Wausau, and Wisconsin Rapids.
