Lakeland Care was one of the first Managed Care Organizations to provide the Family Care benefit in the state more than 24 years ago. We can help meet your long-term care needs.

Our Family Care program is designed to help eligible elders and adults with disabilities live independently – at home and in their community. Family Care provides services and supports to help you live as independently as possible while making sure you are safe and supported at home through natural, volunteer, and paid supports. It also includes care management to help arrange and manage your services and supports. 

Members partner with our care teams and local providers to obtain the support and services they need. As a Family Care member, Lakeland Care will talk with you about what services and supports you need. This might include help with things like bathing, transportation, housekeeping, or home-delivered meals.
