Providers who are contracted with Lakeland Care can access the MIDAS Provider Portal. Once logged in the Provider is required to acknowledge all service authorizations before being able to navigate to any other screen within the portal. This is done by simply checking the box next to Select/Unselect All and selecting save at the bottom of the screen.


If the provider has a question about the Service Authorization or if there is a discrepancy, contact the IDT staff immediately. The IDT staff’s name, phone number, and e-mail address is on the Service Authorization.


Provider Specialists are available in each branch to assist providers and answer any questions about MIDAS.
