Lakeland Care was formed through Creative Care Options, one of the first Managed Care Organizations (MCOs) to pilot the Family Care program in Wisconsin. For over 24 years, we have remained dedicated to providing tailored solutions and support, ensuring we meet the needs of the individuals and organizations we serve effectively and compassionately. Family Care

Lakeland Care embraces a people-centered and community-focused approach to providing long-term care services and support to eligible frail elders and individuals with physical, intellectual, and developmental disabilities through Wisconsin’s Family Care Program. Tribal Care

Lakeland Care currently serves the Menominee Tribe and Oneida Nation in helping to coordinate individualized long-term care. Lakeland Care is an available partner to other American Indian Tribes and Bands within our service regions who are in need. We are committed to upholding the rights and beliefs of those we serve while supporting their personalized needs and outcomes.

Lakeland Care Plus

In our dedication to empowering individuals and strengthening communities, we established Lakeland Care Plus (LC Plus) as a vital resource. LC Plus offers tailored coaching, training, consulting, and Information Technology (IT) solutions, effectively supporting the success of individuals and organizations within our communities.
