Lakeland Care continues to recognize outstanding providers through the C.A.R.E. (Compassion, Accountability, Respect and Enrichment) Award. The C.A.R.E. Award is a way for our care teams to recognize the outstanding service of our providers and individual caregivers. Each quarter an internal Lakeland Care committee chooses three providers/caregivers from all care team submissions and the committee utilizes the C.A.R.E. principles to honor the recognized providers.

Hands with Heart LLC; Jill L.
Hands with Heart was instrumental in helping a member emotionally cope with the loss of her husband, and by visiting her regularly at the hospital. Jill brought her items of comfort such as cappuccino and ice cream. In addition, Jill has been bringing her items from her home, doing her laundry, and assisting with plans for her husband’s funeral. -Nominated by: Nicole T. and Patti B.

Supportive Learning Care; Shannan B.

Supportive Learning Care arranged staffing to ensure a member could return home from the hospital safely with appropriate support. They also completed daily check-ins with her LCI care team while she was hospitalized. -Nominated by: Camille S. and Rachel M.
