Jeremy has been advancing the outcomes of adults who participate in long-term support programs for approximately 15 years, with over 10 years of experience in senior and executive leadership roles. He joined Lakeland Care in 2016 as a Network Relations Manager, and currently serves in the role of Project Manager, where he is responsible for managing internal cross-functional projects, major strategic and enterprise-wide projects, and promoting cross-functional collaboration within all operational units of Lakeland Care. Jeremy has demonstrated unique dedication to long-term care reform in Wisconsin, preparing two counties for transition from legacy waivers and leading our Lakeland Care team through Family Care expansion into a transitioning service area. Prior to joining Lakeland Care, Jeremy served for over seven years as a county human services director, and he is a past president of the Wisconsin County Human Service Association. Jeremy holds a Bachelor’s Degree in Social Work from the University of Wisconsin-Whitewater.
